General Information
As a new patient you may be wondering what will happen at your
first visit to the clinic. It is not uncommon to have some
anxiety if you have never been to a chiropractor or acupuncturist.
In most cases, all patients will leave feeling rested and
relaxed.
At the time of your first visit, we will have paperwork for you to fill out a health history form and acupuncture consent. All forms are located on the top right column and can be filled out ahead of time and brought with you to your first visit. In addition to paperwork a photo id will be copied with your insurance cards for billing purposes.
Please take a moment to review our Notice Privacy Practices located on the top right column. When signing our health history form you agree that you have read and understand our practices.
When you meet with the doctor you may be asked additional questions to give him/her a better idea of the best treatment plan for you. This would be the best time to inform the doctor of any questions you may have or any additional ailments.
After discussing your needs and health history information the treatment will begin. We often ask if you need to use the restroom before hand because of the length of time of the first treatment. You will be asked to lie on a massage table and positioned in the best manner for the acupuncture points to be reached. Common positions are face down, on your back or on your side. If you are uncomfortable please inform the doctor, we would like to make you as comfortable as possible.
Your first visit will last at least an hour and follow up visits are between 15- 45 minutes depending on the patient's treatment plan.

Do I need X-Rays?
If you have an X-Ray or MRI film or
reports please bring them with you at the time of your visit. Not
every patient will need to have X-Ray's taken but the Doctor may
request them to better understand your condition.
How often can I expect to come for treatment?
New patients typically come twice a
week for a couple of weeks, then, depending on how you are responding
to treatment, visits may be decreased to once a week or once a month.
Every body is different and responds differently to treatment.
Regarding Personal injury (PI) or Workers Compensation Cases
Dr. Guo and Dr. Tu does not take any personal injury (PI) cases or worker compensation cases. If you any questions regarding your situation please contact our office.Fee Policy
Acupuncture is not covered by most
insurances. Dr. Guo is not a participating provider of acupuncture
with the insurance companies that provide acupuncture as a benefit.
Therefore, acupuncture is an additional fee.

Self pay Patients
The first new patient visit is a $100.00.
All follow up visits are $70.00.
Chinese herbs and muscle rubs or roll-ons are an additional fee.
Medicare Patients
Acupuncture is not covered by Medicare. Acupuncture is and additional $70.00 per treatment with Dr. Guo.
Chiropractic portion may be billed to Medicare, however chiropractic care is based on medical necessity within Medicare's guidelines and does not cover any maintance care. Please review your Medicare's guidelines to avoid confusion.

Blue Cross & Blue Shield Patients
A photocopy of your current Blue Cross card must be kept in your records. We will submit to the insurance company if you are eligible for chiropractic benefit. Your office visit will be your individual chiropractic copay to cover the chiropractic portion and a additional fee for acupuncture. If you have a deductible associated with your policy you are responsible for payment in full.
